Medicinal plants and herbs have been part of humanity since the dawn of civilization. Neem is a well-known medicinal plant used in India from time immemorial.

The leaf is used for treating eye problem, leprosy, intestinal worms, epistaxis anorexia skin ulcers, and biliousness.

The bark of the tree is used as an analgesic, and used to treat fever.

Neem flower is useful in suppressing bile, treat intestinal worms and phlegm.

Fruit helps in relieving piles, eliminate intestinal worms, treat urinary disorder, epistaxis, expel phlegm, cure eye problems, diabetes, wounds and leprosy.

Neem twigs are used in the cure of cough, asthma, hemorrhoids, intestinal worms, obstinate urinary disorder, and diabetes.

Gum obtained from the bark is a very effective for skin diseases like ringworms, wounds and ulcers, and scabies.

Pulp of the seed and neem oil are used in treating intestinal worms and leprosy.

A combination of the root, bark, leaf, flower and fruit is used in treating blood problems, bile afflictions, leprosy, itching, skin ulcer, urticaria etc.

A paste prepared with neem and turmeric was found to be effective in the treatment of scabies. In 97% of cases, the paste was found to cure scabies within 3–15 days of treatment without any adverse effect.

Dried neem leaves in the form of tea are used by the people of Nigeria and Haiti to treat malaria.

Application of neem oil on hair helps to kill head lice and cure dandruff.

Neem flowers help improve eyesight. For this purpose dry neem flowers are used, preferably 6 months to one year old. These flowers are fried lightly using ghee and then taken internally.

Thus the neem’s role in providing a cheaper, safe solution to modern day diseases is unsurpassed. No wonder it is called “arishta” in Ayurveda which means “reliever of sickness”.

Ginger (Zingiber officinale) is a perennial that grows in India, Jamaica, and China. The plant produces green-purple flowers that resemble orchids. The rhizome (root) is found underground and usually the most valued part of the plant.

Chemical Components

The root contains both volatile and nonvolatile compounds. The nonvolatile constituents, which include the gingerols and gingerol-like compounds, are thought to be responsible for ginger’s flavor, aromatic properties, and any pharmacologic activity. The volatile oil contains zingiberol, zingeberene, curcumene, farnesene, bis-abolene, sesquiphellandrene, and several monoterpenes (linalool, borneol, neral, geraniol, and others). Other compounds present are zingibain (a proteolytic enzyme), oleoresins, fats, waxes, carbohydrates, vitamins, and minerals.

Actions

Human studies have shown that ginger inhibits platelet aggregation induced by adenosine diphosphate and epinephrine . Other studies have demonstrated a lack of effects on platelet aggregation . Ginger extracts have documented anti-inflammatory effects in rodent models . Specific components of ginger produce varying CV effects. Methanolic extracts of ginger have shown positive inotropic effects in a guinea pig model.

Other studies in animals have suggested that components in ginger may be gastroprotective against various chemical insults and stressors. The GI protective action is postulated to be promoted by increased mucosal resistance and potentiation of the defensive mechanism against chemicals or alterations in prostaglandins, providing more protective effects. A study of acetone extracts in mice found them to have similar stimulatory effects on GI motility as those seen with metoclopramide and domperidone .

Reported Uses

Claims for ginger include its use as an antiemetic, an anti-inflammatory useful for arthritis treatment, an antioxidant, an antitumorigenic drug, a CV stimulant, and a GI protectant and as a therapy for microbial and parasitic infestations.

The antiemetic effects of ginger have been extensively studied in humans for morning, motion, and sea sickness and for postoperative nausea and vomiting; most findings provided support for this action. Doses and duration of therapy varied considerably with each study. The antiemetic properties of ginger probably result from local effects on the GI tract rather than the CNS. Increased gastric peristalsis has been shown in animals, but any mechanism in humans is considered speculative.

Ginger has provided relief from pain and swelling in patients with muscle discomfort, osteoarthritis, or rheumatoid arthritis . A proposed mechanism is that it inhibits prostaglandin, thromboxane, and leukotriene biosynthesis.

Dosage

Dosage forms and strength vary with each disease state. As an antiemetic, studies used 500 to 1,000 mg of powdered ginger P.O., or 1,000 mg of fresh ginger root P.O.

Adverse Reactions

CNS: CNS depression (with overdose).

CV: arrhythmias (with overdose).

GI: heartburn.

Interactions

Anticoagulants: Increased risk of bleeding. Avoid administration with ginger.

Contraindications And Precautions

Ginger is contraindicated in pregnant patients; effects are unknown. Some components of ginger have been determined to be mutagenic, whereas others appear to exert an antimutagenic effect. The net effect of these components is unknown . Use only under medical supervision in patients receiving anticoagulants because it may affect bleeding time by inhibiting platelet function.

Special Considerations

Advise women to avoid excessive use of ginger during pregnancy.

Instruct the patient to watch for signs of bleeding when taking ginger.

No consensus exists with respect to dosing and monitoring.

Commentary

Although some data support the use of ginger as an antiemetic in humans, results from several trials have conflicted. Recommendation of ginger for use as an antiemetic, an anti-inflammatory, or a gastroprotective agent, before long-term, controlled, pharmacologic studies of its constituents have been conducted, is premature. Pregnant women should probably avoid excessive consumption of ginger until the effects of all its constituents are understood.

What is Lithotripsy?

Lithotripsy is the use of high-energy shock waves to fragment and disintegrate kidney stones. The shock wave, created by using a high-voltage spark or an electromagnetic impulse outside of the body, is focused on the stone. The shock wave shatters the stone, allowing the fragments to pass through the urinary system. Since the shock wave is generated outside the body, the procedure is termed extracorporeal shock wave lithotripsy (ESWL). The name is derived from the roots of two Greek words, litho, meaning stone, and trip, meaning to break.

Pre-operative care for Lithotripsy:

Prior to the lithotripsy procedure, a complete physical examination is performed, followed by tests to determine the number, location, and size of the stone or stones. A test called an intravenous pyelogram (IVP) is used to locate the stones, which involves injecting a dye into a vein in the arm. This dye, which shows up on X ray, travels through the bloodstream and is excreted by the kidneys. The dye then flows down the ureters and into the bladder. The dye surrounds the stones. In this manner, x rays are used to evaluate the stones and the anatomy of the urinary system. Blood tests are performed to determine if any potential bleeding problems exist. For women of childbearing age, a pregnancy test is done to make sure they are not pregnant. Older persons have an EKG test to make sure that no potential heart problems exist. Some individuals may have a stent placed prior to the lithotripsy procedure. A stent is a plastic tube placed in the ureter that allows the passage of gravel and urine after the ESWL procedure is completed.

The process of lithotripsy generally takes about one hour. During that time, up to 8,000 individual shock waves are administered. Depending on a person’s pain tolerance; there may be some discomfort during the treatment. Analgesics may be administered to relieve this pain.

Procedure of Lithotripsy:

Lithotripsy is a technique used to break up stones that form in the kidney, bladder, ureters, or gallbladder. There are several ways of doing this, although the most common is extracorporeal shock wave lithotripsy. The shock waves are focused on the kidney stone and break the stone into tiny pieces, which are passed out of the body naturally during urination. Throughout the procedure, the doctor can view what is happening to the stones through x-ray or ultrasound monitoring. This procedure prevents patient from having to undergo surgery to have the stones removed, which reduces discomfort, complications, hospital stay, costs, and recovery time.

Patient will usually be asked not to drink or eat anything for 6 hours prior to the procedure, or after midnight of the previous night, if the procedure is in the morning. If patient take regular medicines, patient should ask doctor if they are safe to take before the procedure. For instance, patient may be asked to stop taking aspirin and other drugs that interfere with blood clotting several days before.

On the day of the procedure, patient should wear comfortable clothes that are easy to remove, as patient will have to change into a surgical gown. The procedure generally takes from 45 minutes to 1 hour. In most cases, patient will be asked to lie on a table on top of a soft cushion, through which the shock waves are directed from the lithotripsy machine, called the lithotripter. Older machines require that patient immerse patient in a specially designed tub filled with water, and although less commonly used, these machines are still available today and are effective. Typically, patient will receive some form of anesthesia, and depending on the level of sedation, patient may feel a tapping sensation when the procedure begins. Since lithotripsy can cause mild discomfort, a mild sedative or painkiller is given beforehand. Patient should tell doctor if patient is pregnant, because lithotripsy must not be performed during pregnancy.

While most kidney stones are treated with this type of lithotripsy, not all stones can be treated this way. Sometimes a laser is used to pulverize the stone, but when a laser is used, the doctor must use an endoscope, which is a tube introduced into the body, via the urinary tract, to get close to the stone. The doctor may also access the stone from back into kidney, through a procedure called percutaneous lithotripsy. Laser lithotripsy is usually used when the stone does not respond to extracorporeal shock wave treatment or when it is in a place that is difficult to access. It requires a hospital stay and carries a slightly greater risk of complications than extracorporeal shock wave lithotripsy.

How successful is lithotripsy?

In those patients who are thought to be good candidates for this treatment, about 70 to 90 percent are found to be free of stones within three months of treatment. The highest success rates seem to be in those patients with mobile stones that are located in the upper portions of the urinary tract (kidney and upper ureter). After treatment, some patients may still have stone fragments that are too large to be passed. These can be treated again if symptoms persist.
